Description
Orlistat is a drug designed to treat obesity. It is marketed as a prescription drug under the trade name Xenical by Roche in most countries, and is sold over-the-counter as Alli by GlaxoSmithKline in the United Kingdom and the United States.
Orlistat is the saturated derivative of lipstatin, a potent natural inhibitor of pancreatic lipases isolated from the bacterium Streptomyces toxytricini. However, due to its relative simplicity and stability, orlistat was chosen over lipstatin for development as an anti-obesity drug.
Orlistat is a non-systemic effects of anti-obesity drugs for specific gastrointestinal lipase inhibitor, combined with dietary restrictions can help promote weight loss and prevent weight regain. Gastrointestinal lipases break down fats in the gastrointestinal tract is necessary enzymes, orlistat inhibits stomach, pancreatic and hydroxy acid esterase, primarily through the stomach, small intestine lumen of the stomach and pancreatic lipases active serine parts form a covalent bond, to inactivate enzymes play a therapeutic role. Inactivate enzymes can not be fat in food (mainly triglyceride) hydrolysis of absorbable and non-esterified fatty acid monoacylglycerol. No decomposition of triglycerides (about 30% of the amount of feed) can not be absorbed in the intestine, thereby reducing caloric intake, body weight under control. Systemic absorption of the drug efficacy can play without going through. Orlistat for weight loss, and reduce the risk factors associated with obesity and other diseases associated with obesity (including high cholesterol, type 2 diabetes, impaired glucose tolerance, hyperinsulinemia, hypertension) in morbidity, and reduce the fat content of organs.
Orlistat (also known as tetrahydrolipstatin) is a drug designed to treat obesity. It is marketed as a prescription drug under the trade name Xenical by Roche in most countries, and is sold over-the-counter as Alli by GlaxoSmithKline in the United Kingdom and the United States. Its primary function is preventing the absorption of fats from the human diet by acting as a lipase inhibitor, thereby reducing caloric intake. It is intended for use in conjunction with a healthcare provider-supervised reduced-calorie diet.
The effectiveness of orlistat in promoting weight loss is definite, though modest. Pooled data from clinical trials suggest that people given orlistat in addition to lifestyle modifications, such as diet and exercise, lose about 2-3 kilograms (4.4-6.6 lb) more than those not taking the drug over the course of a year. Orlistat also modestly reduces blood pressure, and appears to prevent the onset of type 2 diabetes, whether due to weight loss itself or to other effects; in a large randomized controlled trial, orlistat was found to reduce the incidence of diabetes by nearly 40% in obese people.
Orlistat is notorious for its gastrointestinal side effects (sometimes referred to as treatment effects), which can include steatorrhea(oily, loose stools). These decrease with time, however, and are the most frequently reported adverse effects of the drug. In the United States and the European Union, orlistat is available for sale without a prescription. Over-the-counter approval was controversial in the United States, with consumer advocacy group Public Citizen repeatedly opposing it on safety and efficacy grounds. Generic formulations of orlistat are available in some countries. In Australia it is listed as an S3 medication, which means it's available over the counter in pharmacies.
History
Despite the fact that orlistat has been commercialized in the US since 1999, Alli was only approved by the FDA in 2007. Since it has half the amount of orlistat than Xenical, it is a less aggressive treatment and may also be used by overweight patients, instead of just obese ones. It is currently the only OTC using orlistat as main ingredient. Alli gained the positive opinion of the FDA, but in 2010, the agency altered the medication’s safety recommendation after cases of severe liver injury in patients treated with orlistat. Another controversial moment marked the history of Alli when in 2014, GlaxoSmith Kline had to withdraw it from the U.S. market due to tampered packages.
Usages
Orlistat is combined with a reduced-calorie diet to promote weight loss.
Orlistat also may be used to prevent weight gain after weight has been lost.
Candidates for treatment with orlistat are patients who are obese with a body mass index (a measure of obesity) of more than 30 kg/m2.
Candidates also include patients with a body mass index of between 27 and 30 kg/m2 if other risk factors for arteriosclerosis are present such as high blood pressure, diabetes, and elevated blood cholesterol or triglycerides.
Based on several scientific studies, the average weight loss that is achieved when orlistat is taken as recommended for six months to one year is 12.4 to 13.4 pounds.
How it works in your body
1. Works by preventing about 25% of fat in the foods that you eat from being absorbed.
2. This undigested fat then passes out of the body, so the number of calories that enter your system is reduced.
3. There is minimal absorption in the bloodstream, which means alli® doesn't affect the cardiovascular and nervous systems.
4. Clinical studies have shown that, for every 2 pounds you lose through diet and exercise, alli® can help you lose one more.
How Do I Take Alli?
Alli comes in capsule form. It’s taken by mouth three times a day, either with a meal that contains a little bit of fat, or up to one hour afterward. If you eat a no-fat meal, your doctor may tell you to skip your dose. Never take more medicine than recommended.
It’s important to follow a low-calorie, low-fat diet while taking this medication. If you eat a lot of fatty foods, even just one high-fat meal like a greasy burger, you’re more likely to have uncomfortable digestive side effects. Choose lean cuts of meat and low-fat dairy products. In general, no more than 30% of your calories at each meal should come from fat.
